The third installment moves away from using Regan as the central character, and in fact claims to be the true sequel to the original exorcist film. It is the 15th anniversary since Father Damien Karras gave his life to save a young girl from hell, something recalled by Father Darra and his friend and investigating cop at the time, Bill Kindermann. It seems that other things currently trouble Kindremann though...a grisly serial killer who calls himself The Gemini Killer. And indeed it isn't long before he is investigating yet another death...that of his friend Father Darra, who upon admittance to hospital is later found with every drop of blood drained from his body and the words 'its a wonderful life' scrawled in blood on the wall.

Kindermann continues to investigate the deaths, and spurred on by the death of his friend vows to bring the killer to justice. He is assisted by the church, who believe that this spate of deaths could be linked to the original exorcism that took place at Prospect Place all them years ago – yet when he receives back the fingerprints found at the scene of his friends death they are revealed to be those of a catatonic patient at the hospital and the plot thickens..it seems that the prints found on other people who have died at the hands of the Gemini killer are all different.

As he probes people at the hospital he is told that there is a man contained at the hospital...a man who was brought in fifteen years ago with total amnesia and no identification. This man has however recently snapped out of his 'state' and claims to be the Gemini killer...and upon seeing him Kindermann suspects the man to be a close friend of his....Father Damien Karras. The rest of the film focuses in on the conversations between Karras, who is clearly stilled possessed by this entity, and Kindermann, who cannot believe that Karras is still alive and furthermore, responsible for the recent spate of deaths.  Meanwhile the deaths continue, a nurse is split down the middle and her organs removed, stuffed with rosaries and sewn back up. Karras, or rather the spirit within, describes how Karras is tormented by what he sees happening, and that it has taken fifteen years for the spirit to regenerate..yet in the meantime it has used others to do its work......catatonics, people that can be easily harnessed.

We are then led to the showdown...the church become involved and want to exorcise this demon once and for all...and so Father Morning, the Lieutenant and Karras/demon are all stuck in a room together...and for the outcome of that meeting I guess you will just have to watch it. A slight improvement on the second installment, but still not a patch on the original.
